You’re Approved! How to Proceed.

If your application for the massage grant program was approved, you received an email directing you to sign the Grant Agreement Form.

Once you have signed the agreement, you can pick a provider using the button below. Staff will notify the provider and send you an email with instructions on how to schedule your appointment within two business days.

You can schedule one massage per month September 9 through December 31.
