We are pleased to announce that open enrollment for MS Center registered clients who are new to the massage program and existing clients who would like to change massage provider, will take place from March 15 through March 31, 2026.

If you are new to the massage program you can complete and sign the Massage Agreement Forms at any of the MS Community Centers with their Site Coordinators. New clients are asked to review the list of available massage therapists and select the therapist they would like to see. Once a selection has been made, please mark it on the Massage Agreement Form so Judi can notify the massage therapist of your choice. Clients are responsible for scheduling their own massage appointments directly with their selected therapist. Visits can be scheduled starting April 1, 2026.

Existing clients who would like to change providers, may email Judi at judi@centers4ms.org. Please note that existing clients may only make a one-time change annually and must allow 5 business days for changes to go through before scheduling an appointment.

Please complete the enrollment form. Once it is received, you will be sent an email with instructions about next steps. You may view the available providers on the Provider List, but do not schedule an appointment with them until you receive approval from us. Note: your enrollment will not be complete until you sign the Agreement Form. 

 Scheduling Appointments

• Do NOT make any appointments until staff notify you that your enrollment is complete.
• Participating clients select a massage therapist from a directory of contracted providers and submit their choice when signing the Grant Agreement. 
• There are providers in each of these areas in Southwest Florida: Bonita Springs/Estero, Cape Coral, Fort Myers, and Naples.
• Once enrolled for 2025, clients contact the therapists directly to make their appointments.
