If you are new to the massage program you can complete and sign the Massage Agreement Forms at any of the MS Community Centers with their Site Coordinators. New clients are asked to review the list of available massage therapists and select the therapist they would like to see. Once a selection has been made, please mark it on the Massage Agreement Form so Judi can notify the massage therapist of your choice. Clients are responsible for scheduling their own massage appointments directly with their selected therapist. 

Existing clients who would like to change providers, may email Judi at judi@centers4ms.org. Please note that existing clients may only make a one-time change annually and must allow 5 business days for changes to go through before scheduling an appointment.

Please complete the enrollment form. Once it is received, you will be sent an email with instructions about next steps. You may view the available providers on the Provider List, but do not schedule an appointment with them until you receive approval from us. Note: your enrollment will not be complete until you sign the Agreement Form.
