With over 500 registered clients and growing demand, providing Care Coordination and Case Management was becoming increasingly difficult for the Centers for Multiple Sclerosis, a Southwest Florida nonprofit serving individuals with MS and their caregivers across multiple counties. To support their efforts, the 25-year-old organization applied for a grant from the Collier Community Foundation to expand the hours dedicated to case management, and to add a Licensed Clinical Social Worker to handle long-term case management for highly disabled clients. The Centers for Multiple Sclerosis is pleased to announce it has received a $30,000 grant from the Collier Community Foundation for the Care Coordination and Case Management Program to help support these vital services.
The Collier Community Foundation awards annual grants through a competitive application and review process. Selected organizations receive grant dollars from the Foundation’s discretionary funds. These funds were gifted by donors who believe in the Community Foundation’s ability to mobilize resources to address the community’s greatest needs.
“Case Management is a core service the Centers for Multiple Sclerosis provides,” explains CEO Kay Jasso. “Clients who are already challenged by a disease that affects mobility, clarity of thought, and expression, can be completely overwhelmed by the complexity of coordination of treatment, various specialty doctors, insurance requirements, medication management, and on top of that day-to-day care and tasks that stress the entire household.”
That’s when Judi LePree steps in. As the Community and Client Services Manager, Judi is the human connection in a world of online forms, voicemail, and phone menus. Judi connects clients to the resources and support they need. The Centers for Multiple Sclerosis are excited about the expansion and quality of services that this grant will provide for all the clients they serve.
Multiple sclerosis is an autoimmune disease that attacks the central nervous system. The Centers for Multiple Sclerosis provide free help in Southwest Florida for those living with this chronic disease. There are an estimated 5,000 people with MS living in Southwest Florida.
